CONNECT Doña Ana is a referral system powered by Unite Us and community health workers designed to connect community members with the essential health and social services offered by our partnering agencies. This system operates as a closed-loop software system, giving individuals the option to enroll voluntarily. Community Health Workers (CHWs) with the Doña Ana County Health and Human Services, are here to help you identify needs and link you to available services. You can also self-refer to the services you need, and our partnering agencies and HHS can monitor the process to ensure your successful connection with the requested supportive services. With this proactive approach, we can help residents of Doña Ana County access services and overcome obstacles preventing them from receiving the care they need.

Our goal is to enhance the well-being of Doña Ana County residents by providing access to health care and social services:

  1. Connecting Doña Ana County residents to health care and social services provided by our partnering agencies.
  2. Preventing individuals from slipping through the gaps in the system.
  3. Monitoring and Tracking referrals until the referral are complete.
  4. Building relationships across agencies to better coordinate care.

 

Budget:

CONNECT Doña Ana is paid by a combination of County and Grant Funds.
